Freedom Hospice One Staff Program
Freedom Hospice's One Staff program is unique in the hospice industry. One Staff is a program of planned communications and protocols designed to improve facility-based patient care, by enabling the facility staff and our staff to interconnect, so that together they act as One Staff.
Sometimes the simplest things in life make the biggest difference - we've all experienced that. In this case, we've discovered that dedicating Freedom Teams to facilities, so that our Teammates really get to know facility team members and the idiosyncrasies of a facility, the performance of the collective team improves for each patient we share.
Building on team-to-team familiarity, we put in place planned communications and protocols so that communication about a patient's condition and actions related thereto no longer happen by "happenstance" but rather, they happen according to plan. That way, we have more effective eyes on the patient and more effective communications about the patient, so that we have more effective actions that benefit the patient. None of this is rocket science, but surprisingly, it is unique in the industry.
